[SCSI] zfcp: Update MAINTAINERS entry

Martin no longer works on zfcp, remove his entry from the MAINTAINERS
file and add Swen as co-maintainer.

Also remove the reference to the zfcpdump.txt file, zfcpdump is not
part of the zfcp device driver.
